The first step in booking your pet's transport is to complete the Client Information Form. This form can be submitted directly from the web site. The contract should be printed, signed and returned to our office via fax or it may be scanned and emailed. The medical release form should be printed and signed.
The Medical Release Form is to be given to the driver along with your pet's health certificate and a copy of the rabies certificate.
